Patients with acute ACL injuries were randomised to either NMES five days a week or a sham treatment, starting soon after injury and continuing through three weeks post-surgery. The result: early NMES use reduced muscle fibre atrophy and preserved contractility at a cellular level — the first study to demonstrate this mechanism directly rather than just inferring it from strength measurements.

One useful nuance from the research: NMES appears to matter most when started early. The systematic review noted that beginning treatment earlier in the post-operative period produces better strength recovery outcomes than starting later. This is a practical point — if you're planning to use a muscle stimulator after surgery, the case for starting in the first week or two, rather than waiting until later in rehab, is stronger than starting late.

What the Evidence Does Not Support

To stay evidence-based rather than oversell the device: NMES is not a passive shortcut to fitness, and it does not replace progressive loading or voluntary strength training for general muscle building in healthy individuals.
